Who Can Refer A Patient?
Who Can Refer A Patient to the Carewell Supportive Care Team?
Physicians can refer a patient by calling the Carewell Supportive Care office at (352) 291-5881.
The office is open from 8-5, Monday-Friday.
What If The Patient Is In A Hospital?
If the patient is in a hospital, nursing home, or assisted living center, physicians may write an order in the facility for a consult.
What Is the Role of the Carewell Supportive Care Team?
Our role is to assess the best management methods for pain and symptom control, which we recommend to the referring physician for follow-up.
